Allow attachments to form

Is it possible to allow someone to upload or attach a file or document when filling out the form.

For example, I have a form for my worship leader to solicit volunteers for auditions. They would like it the person wanting an audition could send along a demo tape of themselves. Or my Media department is working on a print request and needs a piece of artwork from the requester.

Let me clarify somewhat. I am the admin and create the forms. I need the attachments to go to everyone who is listed in the Notifications box. The people who are receiving the form need to receive an attachment.

I know that the administrator can receive the attachment, but it is not necessarily the administrator that wants/needs the attachment. It is the people who the form was created for (like my worship leader).

1) You can add attachments as part of the submission. The File Upload field properties has the option you need to mark in the check-box - "Send File as Email Attachment". That is not set by default.

2) You can add multiple recipients in the email Notification options, as you're probably doing. Separate the email addresses with a comma like email@address.com, email@address1.com, email@address2.com (and so on).

3) You can use conditional logic to send the email notification to specific people, based on responses in the form.

All these functions mentioned have been features of Machform for many years. What version of MF are you using?

Hi there, we are just about to start using this(Attachment) on a form. but it seems that its no way of changing the text, when you do the form it looks good with a Select Files(In Swedish or what ever language you are using), but when you preview or add the form it comes up with a English SELECT FILES and a ugly button. So I looked for how to made the changes in the Themes, but I cant see any easy way of change it. Is it just to CSS changes.

What I´m looking for I guess is like the "Form Submit Button", which is easy way of change to different languages.

Background, we are using the forms on multiple websites and then of course on different languages. And also we are using "SELF-HOSTED" version of your wonderful "formcreator".
