Have a special field type for NPS, it's a common question type these days and current settings do't allow you to build a nice looking form (that works on desktop browser and mobile)
It's just 10 radio buttons in line with the labels aligned and looking nice on 1 and 10 only. "Note likely at all" and "Extremely Likely"
