Don't know what you mean by "template". You just insert the fields into your new form each time. There are different varieties of the "Name" field and the Address field is configurable.
Maybe, what you can do, is design the form and save a "master" copy (it can remain unpublished/inactive), and then each time you need a new one, create a copy of that first one and rename it as the "real" form, you then have all the the pre-existing fields you need, and just add others as required.
