I am running into a weird issue and not sure how to fix it.
Upon creating a new form and clicking on Submit, I just get a message saying "Saving..." and it stays like that. When I go to manage forms page, it gives me an error like:

Query Failed: SQLSTATE[42S02]: Base table or view not found: 1146 Table 'membershipforms.ap_form_33863' doesn't exist
Because of that error, I can't view the manage forms page, however, if I go an individual form page (if I know the url), I can visit that page.
I am not sure what changed or what could have caused this issue to happen.
Upon checking the database, I see that form record is added in the ap_forms table but no table for that form is created. To troubleshoot it, I created a fake table called ap_form_33863, after which the error goes away and I can see the manage forms page.
Now if I try to add any fields in the form, it keeps saying "Saving.." again but doesn't actually add any fields.
I tried to delete that newly created form entry from the ap_forms table to see if that form is faulty, but again get the same error when I create a new form.
I tried googling it but didn't find much helpful so hoping that someone here will be able to help me out.
