We've been using Machform at our community college for going on close to 9-10 years now. If I remember correctly, one of the first (or perhaps the first) form we created that made use of the signature pad in Machform was an application we created for our EOPS program and that one is still being used today. There are other forms where we collect info from staff/faculty/students (e.g. voting on CTA Tentative Agreements, Associated Student Government Elections, Scholarship Applications, etc.) but we don't always make use of the signature pad on those (for those situations that don't require it we usually make use of a description field describing what's being agreed to/submitted along with a checkbox of some sort signifying agreement that they would be required to check off). For a while we also used it as a makeshift job applications system since we were still primarily doing paper forms before moving over to NeoGov recently.
I don't know if the areas taking advantage of the signature pad are collecting a separate wet signature in person from the students at all (my guess is not).
